Official Notice agrees to provide live music for duration of ________ hours, with one 10 to 15 minute break per hour. Pre-recorded music will be provided during breaks. Any additional time will be charged at our normal hourly rate of ____________ per hour.
Down Payment: __________ has been paid. This guarantees and holds the customer's engagement date. The remainder of the balance will be due on the engagement date.
Cancellation: The down payment shall be 100% refundable as long as the date is no less than 60 days before engagement. Any cancellation that occurs after the 60 days prior to engagement is non-refundable. If Official Notice cancels for any reason, down payment shall be refunded in full.
